OHIO ACADEMIC COMPETITION
ENTRY AGREEMENT

In consideration of the agreement to permit the ______________________________________
High School academic team to participate in the Ohio Academic Competition, I, ____________________________, coach, adviser, participant, reader, moderator, or team member, of said team do hereby release and forever discharge the Ohio Academic Competition, and their respective officers, directors, members, and assigns, and each of them, from any and all claims, demands, damages, actions, and causes of action of every name and nature, anticipated or unanticipated, which I may have against any of the aforementioned organizations and persons, which may arise out of, result in, or any manner be connected with the tournament competition in which I participate as a coach, adviser, participant, or team member.

Furthermore, I agree to follow all rules of the competition as outlined in the official rules distributed in the registration packet. I understand that any unsportsmanlike behavior on the part of any team member or adviser may be grounds for disqualification from the tournament by the tournament officials who include readers, moderators, and/or the tournament director.

Furthermore, I understand that the decisions of the judges and the appeals board are final and may not be appealed. I agree to comply with such decisions.
